What is In-Home Outpatient Therapy?
It means I come to you! Wells Rehab & Wellness provides physical therapy and wellness services in the comfort of your own home. Yes, I make house calls! You don't need to be "homebound" to qualify, you'll receive one on one treatment sessions, it is covered by insurance and with direct access in Michigan you may not even need a prescription.
Who Is Appropriate For Services?
Recent illness, injury, surgery
History of fall(s) or fear of falling
Decline in functional mobility or abilities
Discharged from a hospital, rehab center, or home health care
Recently moved into an assisted or independent living facility
Anyone wanting to start an exercise program
Over 20 years of providing physical therapy in multiple settings in addition to active membership in the American Physical Therapy Association(APTA), APTA Geriatrics and APTA Michigan allows me to offer current, evidence-based care.
I received my Master of Science in Physical Therapy from Central Michigan University in 2002, am CPR/AED certified, and have advanced knowledge in geriatrics and neurology including:
Training in the Otago Exercise Program
Certified Exercise Expert for Aging Adults (CEEAA)
Certified Brain Injury Specialist (CBIS)
Certified Dementia Practitioner (CDP)
I am APTA Geriatrics' Michigan Advocate and the lead organizer of the Senior Athlete Fitness Exam at the Michigan Senior Olympics. I am a past recipient of APTA Michigan President's Award and have served on APTA Michigan's Board of Directors. I have been employed as a PT and/or in leadership/management positions at various hospitals, rehab centers, and geriatric focused out-patient clinics.
Wells Rehab & Wellness services the majority of Oakland county, as well as parts of Livingston and Wayne counties.
Wells Rehab accepts Medicare, Blue Cross Blue Shield of Michigan, BCBS Medicare Advantage/Medicare Plus Blue, and any other Medicare Advantage Plan with out of network benefits. Wells Rehab will provide a "Good Faith Estimate" explaining your costs prior to initiating any service if requested.
Other services provided by Wells Rehab:
- Contracting with home health and hospice agencies to provide PT services
- Rehab consulting services including chart audits, staff education, and program development
- Presentations on Fall Risk & Prevention to local senior centers, senior living communities, and various other organizations (free of charge)
